MEDICAL FILE
Good oral care requires a medical record for every patient. The file consists of notes about the dental condition, details of performed and planned treatments, reports of examinations, correspondence with other dentists and doctors by whom you have been referred or to whom we refer you. Data is also included that, after your permission, has been requested from other healthcare providers, for example your GP.
Data from the medical file can be requested by other healthcare providers with your permission. This will not happen if you object to it. A limited part of the file is used for financial administration.
You have the right to inspect and obtain a copy of your data. If you indicate that data needs to be corrected or added, then this is possible.
PROVISION OF YOUR PERSONAL DATA TO THIRD PARTIES
In principle, we only provide your medical and personal data to third parties if you have given permission for this yourself. Data is provided without your consent if this is necessary for the execution of our healthcare agreement or if there is a legal obligation.
STORAGE PERIOD
We do not store your data for longer than is necessary for the purposes stated in this privacy statement and for our legal obligations. We keep your medical data for 15 years, in accordance with the Medical Treatment Agreement Act.
SECURITY MEASURES
To protect your data as best as possible, we have taken physical and digital security measures. Your medical file can only be viewed by PC Hooft Clinic employees.
